Service Overview

The community-based weight management service triages and treats eligible patients in the community. Not all patients will be eligible to receive Tirzepatide/Mounjaro on the NHS.

Consents

To use this service you will need to agree to the following statements:

• You agree for us to have access to your medical record.
• You are willing to travel to your first appointment.
• You understand that you need to provide appropriate identification at your first appointment (e.g passport, driving license, etc…)
• You agree to commit to a 9-month 'wrap around care' treatment plan which offers advice on lifestyle, diet and exercise.
